Our Story
Evolve Transition Support is a disability support service dedicated to empowering young adults aged 16-35 to transition into adulthood, build meaningful community connections, and live with purpose and autonomy.
We offer personal care, community access, and daily living support—but our work goes far beyond tasks. We’re here to walk alongside young people as they navigate one of life’s most significant transitions, helping them discover their place in the community and develop the skills and confidence to thrive.
Our Why
We founded Evolve Transition Support because we believe people with disabilities deserve support that honours their voice and centres their lived experience. Too often, services are designed for people rather than with them. We’re changing that. Our approach is built on genuine co-design, working directly with disabled people and their friends and families to shape services that reflect real needs, real goals, and real lives.
We believe the problem is never the person, but rather the barriers society puts in their way. Our mission is to dismantle those barriers and create pathways to full and equal participation in commpunity life.
